• New client questionnaire (Connie will send this with portal access)
• Bank information for direct deposit or automatic withdrawal — include a voided check
• Copy of your driver’s license (an IRS requirement to e-file)
• For each dependent: copy of Social Security card and birth certificate
• Dependents under 17: school records, medical bills, or other documents showing name, address, and who claims the child
• Form 1095 verifying health insurance
• A written list of questions you want answered
